Community Enablement Support (CES)

Community Enablement Support

Aims to Improve your Independence.

We support people with mental health and or substance misuse needs within primary care through skills gains programmes and time limited interventions. We work alongside multi-disciplinary mental health teams and occupational therapy led services, as well as a diverse range of community agencies and services. Our Service supports people in their recovery with prevention; working with them to maximise their choices and independence in line with our commitment to the personalisation agenda.  Clients and their family are involved in  the  development of support plans as well as contribute to reviews and assessments to maximise independence and recovery.

We agree goals that work to your strengths and develop a plan to improve your life skills and confidence. Our Worker will work with you on your goals for up to 12 weeks.

Community Forensic Enablement support.

Aims to Improve your Independence.

At Kent Social Care and Consultancy, we provide specialist communitybased support for people who have an offending history or are at significant risk of offending. Many individuals arrive from secure hospitals, criminal justice settings or unstable placements, often under Ministry of Justice restrictions or Mental Health Act conditions. We offer them stability, purpose and a clear pathway to a safer, more independent life.

Our forensic model is founded on the internationally recognised Good Lives Model, which focuses on each person’s strengths, aspirations and potential. Rather than focusing solely on managing risk, we work to build capabilities, confidence and longterm wellbeing. Our approach is about enabling growth — creating opportunities for a positive future instead of repeatedly revisiting past challenges.

Key aspects of our model include:

  • Personcentred planning driven by strengths, wellbeing and personal goals.
  • Comprehensive forensic risk assessment and management, shaped through multidisciplinary collaboration.
  • Skilled Positive Behaviour Support, traumainformed practice and strong mental health expertise.
  • Support that promotes meaningful relationships, purposeful activity, independence and community inclusion.

Above all, we see each individual as a person first — not an offender and not a diagnosis. This respectful, empowering ethos underpins every part of our work and ensures that people can move forward with dignity, confidence and hope.
